Role Overview:
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct in-home health assessments for Medicare Advantage members to improve health outcomes through education, gap closure, and collaboration with primary care providers.
Responsibilities:
Perform comprehensive assessments, including history review, vital signs, physical exams, and screenings
Document diagnoses and communicate findings to PCPs
Identify urgent/emergent issues and intervene when necessary
Educate patients on disease management and medication adherence
Address social determinants of health and provide referrals
Qualifications & Skills:
Active, unrestricted licensure in practicing state and relevant certifications (ANCC, AANP, NCCPA)
Reliable transportation and ability to navigate home environments
Proficiency with electronic medical records and technology
Preferred: 1+ years clinical experience, experience with geriatric or Medicare populations
