Role Overview:
As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform in-home health assessments for Medicare Advantage and other plan members to improve health outcomes through education, gap closure, and collaboration with primary care providers.
Key Responsibilities:
- Conduct comprehensive assessments, including medical history, vital signs, physical exams, and screenings
- Identify diagnoses for care management and communicate findings to PCPs
- Recognize urgent situations and intervene appropriately
- Educate members on health conditions, medications, and adherence
- Address social determinants of health and provide referrals
Qualifications & Skills:
- Active licensure and certification (ANCC, AANP, NCCPA) in relevant specialties
- Unrestricted license and driver’s license with reliable transportation
- Ability to navigate home environments and perform assessments physically
- Preferred: 1+ years clinical experience, proficiency with EMRs, strong communication skills with older and Medicare populations
