Seeking an Infectious Disease Nurse Practitioner (NP) in the Chicagoland area for part-time or full-time, in-person roles at multiple skilled nursing and long-term care facilities.
Provide comprehensive assessments, diagnose, treat, and manage infectious diseases, prescribe medications, and develop individualized care plans.
Collaborate with care teams, guide residents’ plans of care, and coordinate referrals and discharge processes.
Maintain documentation, address care gaps, and ensure quality outcomes with a minimum of 12 patient visits per day.
Qualifications include an active Illinois NP license, preferably with infectious disease and long-term care experience.
