Role Overview:
Seeking a bilingual (Spanish-speaking) Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments across Massachusetts. This field-based role focuses on improving patient outcomes through comprehensive evaluations, education, and care coordination without on-call or weekend duties. The position involves approximately 85% travel with all expenses covered, offering flexible scheduling between 8am-8pm, supporting work-life balance.
Key Responsibilities:
- Perform thorough in-home assessments, including medical history, vital signs, and screenings
- Identify diagnoses and document findings for care management
- Communicate with primary care providers to address care gaps
- Recognize urgent situations and provide appropriate interventions
- Educate members on health conditions, medications, and social needs
- Provide social determinants of health referrals
Qualifications & Skills:
- Active licensure and certification (ANCC/AANP for NPs)
- Unrestricted driver’s license and reliable transportation
- Ability to navigate diverse home environments and conduct physical assessments
- Preferred: 1+ years clinical experience, proficiency with EMRs, strong communication skills with geriatric/Medicare populations
Compensation & Benefits:
Salary ranges from $109,500 to $164,000 plus incentives, with comprehensive benefits, recognition programs, and retirement options. Performance-based rewards and development opportunities are available.
