Role Overview:
The HouseCalls Advanced Practice Clinician conducts in-home health assessments for Medicare Advantage members, focusing on improving health outcomes through education, gap closure, and collaboration with primary care providers. This non-prescribing, field-based role involves reviewing medical history, performing physical exams, screenings, and point-of-care testing, and identifying diagnoses for care management. The clinician communicates findings to PCPs, recognizes urgent situations, educates patients on disease and medications, and addresses social determinants of health with referrals as needed.
