We are seeking a Hospice Nurse Practitioner to provide palliative care, symptom management, and supportive services to hospice patients.
Key responsibilities include conducting face-to-face visits, collaborating with the Hospice Medical Director and other healthcare professionals, developing and coordinating care plans, documenting patient progress, and instructing patients and families in self-care techniques.
Qualifications include an active unrestricted license in the state of practice, current certification as a Nurse Specialist or Nurse Practitioner, national certification (e.g., ANCC, NBCHPN, AANP), and at least 5+ years of nursing experience. A valid driver’s license, dependable transportation, and CPR certification are also required.
Preferred skills include experience in oncology, geriatrics, hospice, or intensive care, and familiarity with interdisciplinary teams.
