Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs.

Deliver comprehensive assessments, diagnose, and manage acute and chronic conditions.

Order diagnostics, develop treatment plans, and prescribe medications within guidelines.

Educate patients, families, and staff on health and prevention.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality initiatives.

Qualifications include current MN NP license, board certification, and preferred experience in geriatric or post-acute care.
