Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, LTC, and ALFs.

Deliver patient assessments, manage acute/chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality initiatives.

Qualifications: Minnesota NP license, Board Certification, 1+ year experience preferred, strong geriatric and primary care background.
