Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner for a mobile primary care role in skilled nursing, long-term care, and assisted living facilities.

Provide comprehensive primary care, manage acute and chronic conditions, and build relationships with residents, families, and staff.

Perform assessments, order diagnostics, develop treatment plans, and prescribe medications within guidelines.

Collaborate with care teams to ensure seamless, patient-centered care and support quality improvement efforts.
