Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in Skilled Nursing Facilities, LTC, and ALFs.

Perform assessments, exams, and chart reviews; develop and adjust treatment plans.

Manage acute and chronic conditions, prescribe medications, and interpret diagnostics.

Educate patients, families, and staff to promote health and prevention.

Collaborate with team members to ensure seamless care and support quality improvement.

Qualifications: Minnesota NP license, Board Certification, 1+ year experience preferred, strong geriatric and primary care background, excellent communication skills.
