Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, diagnose, and manage acute and chronic conditions.

Order diagnostics, develop treatment plans, prescribe medications, and educate patients and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality improvement efforts.

Qualifications: Minnesota NP license, board certification, with 1+ year experience preferred. Strong geriatric and primary care skills required.
