Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform assessments, physical exams, and develop treatment plans for acute and chronic conditions.

Order and interpret diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications: Minnesota NP license, board certification, 1+ year experience preferred, strong geriatric and primary care background, excellent communication skills.
