Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.

Perform patient assessments, order diagnostics, manage acute and chronic conditions, and develop treatment plans.

Prescribe medications, educate patients and families, and collaborate with care teams to ensure seamless care delivery.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications include a Minnesota NP license, board certification, and preferably 1+ year of experience in geriatric or post-acute care. Strong communication and teamwork skills are essential.
