Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

We are seeking an experienced Nurse Practitioner for a mobile primary care role in Skilled Nursing Facilities, LTC, and ALFs.

Provide comprehensive primary care, manage acute and chronic conditions, and build strong relationships with residents and care teams.

Perform assessments, order diagnostics, develop treatment plans, and prescribe medications within guidelines.

Collaborate with facility staff to ensure seamless, patient-centered care.

Qualifications: Current Minnesota NP license, Board Certification, 1+ year experience preferred, strong geriatric and primary care background.
