Requirements:
- Certified Nurse Practitioner license
- Preferred 2+ years in Skilled Nursing facilities
- Ability to work in a fast-paced environment

- Ability to travel within a 12-20 mile radius
Responsibilities:
- Collaborate on treatment plans with patients and teams
- Perform assessments, diagnoses, and treatments
- Conduct weekly rounds in assigned facilities
- Maintain accurate medical records
