We are seeking a dedicated part-time Nurse Practitioner to provide direct patient care in Skilled Nursing, Assisted Living, and Rehab facilities. Working closely with an attending MD, you will evaluate and treat patients, coordinate care, and ensure compliance with facility and PSG guidelines. Your responsibilities include managing admissions, follow-ups, lab reviews, and discharge planning, while maintaining thorough documentation and billing. The role requires strong clinical skills, team collaboration, and the ability to work independently within a M-F, 8a-4p schedule, three days a week.
