Nurse Practitioner – Live Well Homecare

We are seeking a dedicated Nurse Practitioner for primary care and inpatient work, offering a flexible M-F 9-5 schedule with no weekends or on-call responsibilities.

Provide primary and preventive care to residents in skilled nursing facilities.

Document evaluations, diagnoses, and treatment plans accurately.

Assess and manage acute patient conditions promptly.

Prescribe medications, order labs, and follow up on diagnostics.

Manage patient visits via EMR, dashboards, and telehealth as needed.

Collaborate with staff and PCPs on patient care.

Complete documentation and billing accurately and timely.

Qualifications include active NP license, board certification, DEA, ACLS/BLS, and malpractice certification.

Compensation ranges from $120,000 to $130,000 annually, with benefits such as health insurance, continuing education, paid time off, bonus opportunities, and more.
