Role Overview:
Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage and other members. The role focuses on improving health outcomes through education, gap closure, and collaboration with primary care providers, without prescribing authority. Responsibilities include comprehensive assessments, documenting diagnoses, communicating with PCPs, recognizing urgent situations, and addressing social determinants of health.
Key Responsibilities:
Perform medical history reviews, physical exams, screenings, and point-of-care tests
Identify diagnoses and document for care management
Coordinate with PCPs to address care gaps
Educate members on health topics and medications
Address social factors impacting health and provide referrals
Recognize urgent issues and intervene appropriately
Qualifications & Skills:
Active, unrestricted licensure in practicing state
Valid driver’s license and reliable transportation
Ability to navigate varied home environments and perform assessments
For NPs: Certification from ANCC or AANP; PAs: NCCPA certification and state licensure
Preferred: 1+ years clinical experience, proficiency with EMRs, ability to transport equipment, strong communication skills with diverse populations
