Role Overview:
Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage members across Washington. This field-based position involves evaluating medical history, performing physical exams, screenings, and identifying care gaps. The role emphasizes patient education, addressing social determinants of health, and collaborating with primary care providers to improve health outcomes.
Responsibilities:
- Perform comprehensive in-home assessments, including history, exams, and screenings
- Document diagnoses and communicate findings to PCPs
- Recognize urgent situations and provide appropriate interventions
- Educate patients on disease management and medication adherence
- Address social factors impacting health and provide referrals
Qualifications & Skills:
- Active, unrestricted licensure in Washington
- Certification: ANCC/AANP for NPs or NCCPA for PAs
- Reliable transportation and ability to navigate varied home environments
- Preferred: 1+ years clinical experience, proficiency with EMRs, strong communication skills with Medicare populations
Benefits & Compensation:
Offers base salary, incentives, comprehensive benefits, and performance bonuses. The role provides flexible scheduling, travel expenses covered, and no on-call, weekends, or holidays. Compensation ranges from $109,500 to $164,000, depending on experience and performance.
