Role Overview:
Seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care gap closure. The role involves conducting comprehensive evaluations, documenting diagnoses, communicating with primary care providers, recognizing urgent situations, and addressing social determinants of health through referrals.
Responsibilities:
- Conduct medical history reviews, physical exams, vital signs, and screenings
- Identify and document diagnoses for care management
- Collaborate with PCPs to address care gaps
- Recognize and intervene in urgent/emergent situations
- Educate members on health conditions, medications, and compliance
- Address social factors impacting health and provide referrals
Qualifications & Skills:
- Active, unrestricted licensure and certification (ANCC/AANP for NPs, NCCPA for PAs)
- Valid driver’s license and reliable transportation
- Ability to navigate home environments and perform physical assessments
- Preferred: 1+ years of clinical experience, proficiency with EMRs, and strong communication skills with geriatric or Medicare populations
