Role: Medical Social Worker (MSW) in Home Care.
Responsibilities:
- Conduct psychosocial assessments, counseling, crisis intervention, and provide information/referral services.
- Collaborate with management to optimize resource utilization for patient outcomes.
- Assist patients and families in understanding and accepting care recommendations to promote social and health adjustment.
- Stay informed on community resources for social, emotional, and financial support.
- Identify and document barriers to healthcare transition, including clinical, psychosocial, and financial issues.
- Participate in audits to ensure compliance with laws and guidelines.
- Work with patients and support systems to develop and implement care plans.
- Perform initial psychosocial evaluations through patient interviews.
- Report suspected cases of abuse or neglect.
Qualifications & Skills:
- Master’s Degree in Social Work.
- Minimum 1 year of social work or mental health experience.
- Effective communication with diverse populations and adherence to NASW Code of Ethics.
- Knowledge of patient rights, laws (like HIPAA), and psychological principles.
- High emotional intelligence, empathy, and reasoning skills.
- Preferred: LSW license in Pennsylvania.
