Role: Medical Social Worker (MSW) in Home Care, responsible for providing psychosocial assessment, counseling, crisis intervention, and referral services.
Key Duties:
- Conduct initial psychosocial evaluations through patient interviews.
- Employ counseling techniques to help patients and families understand, accept, and follow care recommendations.
- Intervene effectively in crisis situations using crisis intervention strategies.
- Identify and document psychosocial, clinical, and financial barriers to healthcare transitions.
- Collaborate with management and healthcare teams to optimize resource utilization for positive patient outcomes.
- Stay informed about community resources for social, emotional, and financial assistance.
- Report suspected cases of abuse, neglect, or exploitation.
- Participate in audits and reviews to ensure compliance with laws and regulations.
Qualifications & Skills:
- Master’s Degree in Social Work.
- At least 1 year of social work or relevant mental health experience.
- Effective communication skills and ability to work with diverse populations.
- Knowledge of patient rights, laws such as HIPAA, and psychological principles.
- High emotional intelligence and empathy.
- Ability to logically analyze and solve problems.
- Preferred: Licensed Social Worker (LSW) in Pennsylvania.
Additional Information: Must be able to lift 7 lbs., sit for extended periods, and perform repetitive motions. The role emphasizes teamwork, compliance, and compassionate patient care.
