The Medical Social Worker (MSW) provides psychosocial assessments and supports patients and families in the community receiving Home Care and Hospice services. As part of the interdisciplinary team, the MSW guides patients through illness, long-term care planning, and connecting to community resources.

Responsibilities:
- Assess psychosocial status and communicate findings
- Develop intervention plans based on evaluations
- Provide information, referrals, and act as liaison with community agencies
- Participate in care planning and discharge processes
- Support the healthcare team with social and emotional insights
Qualifications:
- Master’s degree in social work from an accredited program
- Minimum one year healthcare experience
- Strong communication skills
- Proof of vaccine, reliable transportation, valid driver’s license, insurance
- Recent physical exam and TB test
