We seek a Medical Social Worker (MSW) to conduct psychosocial assessments and serve as a resource for patients and families receiving Home Care and Hospice services. As part of an interdisciplinary team, the MSW supports illness navigation, long-term planning, and community resource connection.

Assess psychosocial status and communicate findingsPlan interventions, provide referrals, and serve as liaisonParticipate in care planning and discharge processesSupport team with understanding social/emotional factors**Qualifications:**
