Role Overview

The Licensed Practical Nurse (LPN) Medication Nurse provides safe and accurate nursing support, focusing on medication administration, health monitoring, and clinical documentation to ensure resident safety and well-being.

Key Responsibilities:

• Administer medications and treatments according to physician orders while following the Six Rights of Medication Administration
• Accurately document in MAR/TAR in real-time
• Monitor symptoms, report adverse reactions, and support symptom relief through medication management
• Conduct Service & Health Updates (SHUs) and transcribe physician orders for care plan updates
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards

Qualifications & Skills:

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment, documentation, and clinical skills
• Experience in senior care or medication administration preferred

This role is critical in preventing medication errors, early detection of health changes, and safeguarding resident health through clinical precision and adherence to regulations.
