Role: Licensed Practical Nurse acting as a Patient Navigator to guide new patients, provide education, and ensure continuity of care.
Responsibilities: Assess patient needs via phone or documentation, coordinate care with healthcare providers, communicate effectively with patients, manage program activities, and serve as a liaison for scheduling and support services. Perform medical record reviews, assist physicians, maintain clinical supplies, and ensure proper functioning of equipment. Develop screening tools and procedures to enhance patient navigation and address patient concerns related to access and billing.
Qualifications: Minimum of 2 years LPN experience, graduate from Specialty Training Program in Nursing, and clinical knowledge to facilitate patient entry and ongoing support.
Skills: Strong communication, collaboration, organization, and clinical skills to support diagnosis, treatment, and patient education.
Work Environment: Primarily in a clinical setting, adhering to organizational policies and procedures.
