LPN Medication Care Manager – Senior Living

Role Overview

The LPN Medication Nurse provides licensed nursing support by administering medications, monitoring residents’ health, and maintaining accurate clinical documentation in a senior living environment.

Key Responsibilities:

  • Administer medications and treatments according to physician orders, following the Six Rights of Medication Administration.
  • Accurately document in MAR/TAR in real time and transcribe physician orders.
  • Monitor symptoms, report adverse reactions or refusals, and conduct Service & Health Updates (SHUs).
  • Observe and report physical, emotional, or cognitive changes in residents.
  • Maintain medication carts, complete narcotic counts, and ensure regulatory compliance.

Qualifications & Skills:

  • Active LPN license in good standing.
  • CPR and First Aid certification.
  • Strong assessment, observation, and documentation skills.
  • Experience in senior care or medication administration preferred.
