Life Plan Counselor provides compassionate guidance to older adults and their families as they evaluate senior living options.
Responsibilities
- Listen to individual needs and help determine appropriate Life Plan and continuing care choices.
- Educate prospective residents and families on available services and the decision-making process.
- Work collaboratively with teams across independent living, assisted living, memory care, and skilled nursing to support smooth, informed transitions.
- Ensure a resident-first experience with professionalism, empathy, and respect.
