Licensed Social Worker – Ambulatory

Role Summary:


Provides comprehensive social work services including assessment, care planning, case management, and intervention to patients and families. Acts as a liaison between healthcare providers and community resources to coordinate care, promote communication, and enhance patient support through public relations activities.


Key Responsibilities:

  • Conducts psychosocial assessments, develops, and implements social work care plans, addressing emotional, mental, and behavioral health issues.
  • Performs case management, resource referral, advocacy, and mediates to ensure appropriate service delivery and patient placement.
  • Participates in program development, policy implementation, quality improvement, and departmental activities such as training and record audits.
  • Engages in research, documentation, and compliance with regulations, ensuring accurate medical records.

Qualifications & Skills:

  • Master’s degree in social work.
  • Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
  • Basic Life Support certification.
  • Preferred: minimum one year of social work experience, healthcare background, and community networking skills.
