Role: Licensed Practical Nurse serving as a patient navigator to guide new patients, provide education, and ensure continuity of care.
Responsibilities: Facilitate patient introduction to the program, evaluate patient needs via phone or documentation, collaborate with healthcare providers to coordinate care, communicate with patients, and serve as a point of contact for providers. Coordinate patient activities, provide education, maintain systems for follow-up, develop screening tools, investigate patient concerns, and assist with medical record reviews and clinical procedures. Ensure proper operation of equipment and maintain clinical environments.
Qualifications & Skills: Minimum of 2 years of LPN work experience, graduate from Specialty Training Program-Nursing, strong clinical knowledge, teamwork, communication, and organizational skills.
Key Duties: Deliver clinical support, assist in diagnosis and treatment, develop and implement patient navigation procedures, maintain documentation, and support interdisciplinary collaboration in a clinical setting.
