Role Overview: The Licensed Practical Nurse (LPN) serves as a patient navigator, guiding new patients, providing education, and supporting ongoing care.
Key Responsibilities:
- Facilitate patient entry into the program via calls and documentation review.
- Collaborate with healthcare providers to coordinate and implement care plans.
- Communicate with patients through various channels, ensuring timely information exchange.
- Coordinate patient activities, support groups, and educational resources.
- Conduct medical record reviews and assist with clinical procedures.
- Maintain clinical supplies, equipment, and exam rooms.
- Investigate patient concerns related to appointments, communication, and billing.
- Develop screening tools and efficient clinic procedures.
- Serve as a liaison with scheduling and support services.
Qualifications & Skills: Minimum of 2 years LPN experience, clinical knowledge, strong communication, teamwork, and organization skills.
Education: Graduation from a Specialty Training Program in Nursing is required.
