Licensed Practical Nurse (LPN) will deliver high-quality patient care across long-term care, skilled nursing, and post-acute facilities.
- Provide direct patient care and support patients with daily clinical needs
- Administer medications safely and according to clinical protocols
- Monitor patient conditions and report changes to the care team
- Assist with implementing and supporting individualized care plans
- Document patient observations, medication administration, and care provided in an EHR
- Collaborate with interdisciplinary teams to ensure continuity of care
Qualifications & Skills
- Active LPN license
- Strong medication administration and patient monitoring skills
- Accurate, thorough documentation in electronic health records
- Professional, compassionate bedside manner and teamwork orientation
