Licensed Practical Nurse (LPN)
Provide compassionate, resident-centered care in an assisted living setting. Monitor health status, administer medications, and perform assigned treatments. Document all care accurately in an EHR system and maintain clear, timely records.
Key duties include:
- Assess residents’ condition and promptly respond to changes
- Administer medications and complete prescribed treatments
- Maintain accurate documentation of nursing care in EHR
- Collaborate with RNs, physicians, and caregivers to support individualized care plans
- Promote resident safety, wellbeing, and adherence to care standards
Qualifications & skills:
- Current LPN license
- Strong clinical judgment and patient-focused communication
- Ability to work effectively in a team environment
- Comfort with EHR documentation and care tracking
