Role: Licensed Practical Nurse acting as a patient navigator, guiding new patients, providing education, and supporting ongoing care.
Responsibilities: Facilitate patient intake, assess needs via phone or documentation, coordinate care with healthcare providers, communicate with patients, and serve as a point of contact for providers.
Assist with patient activities, educational programs, and support services, while maintaining patient records, scheduling, and inventory. Conduct medical record reviews, provide nursing care, and ensure equipment and facilities are properly maintained.
Qualifications & Skills: Minimum 2 years LPN experience, with preferred cardiovascular and adult echocardiography background. Strong communication, collaboration, and organizational skills required.
Additional Details: Full-time, 40 hours/week, day shifts. Must have graduated from a Specialty Training Program in Nursing.
