Seeking a compassionate Licensed Practical Nurse (LPN) to deliver resident-centered care in long-term care and post-acute settings. The LPN provides hands-on nursing, administers medications and treatments, monitors residents’ conditions, and documents care in the EHR. Collaborates with an interdisciplinary team to support safe, high-quality outcomes while maintaining dignity and patient safety.
Key responsibilities:
- Administer medications and perform prescribed treatments
- Monitor health status and report changes promptly
- Document nursing care and observations in the EHR
- Collaborate with interdisciplinary team members
- Support residents and families through empathetic, respectful care
- Mentor and assist CNAs to ensure consistent care standards
Qualifications/skills:
