Licensed Marriage and Family Therapist

Role: Licensed Marriage and Family Therapist (LMFT) providing in-person psychotherapy to the geriatric population in skilled nursing, assisted living, and sub-acute facilities.

Responsibilities:

  • Conduct individual and group therapy sessions, assessing, diagnosing, and treating emotional and behavioral health conditions such as depression, anxiety, and cognitive decline.
  • Develop and implement evidence-based treatment plans in collaboration with facility staff, families, and interdisciplinary teams.
  • Maintain accurate clinical documentation using a user-friendly EHR system, monitor patient progress, and adjust treatment as needed.
  • Work within regulatory standards and provide compassionate, patient-centered care in a supportive environment.

Qualifications & Skills:

  • Active, unrestricted LMFT license in the applicable state.
  • Effective communication skills, organizational ability, and proficiency with electronic health records.
  • Ability to provide in-person services within a 45-minute radius, with physical mobility to move throughout facilities.
  • Full vaccination and booster may be required in some settings.
