Role: Licensed Marriage and Family Therapist (LMFT) providing in-person psychotherapy to the geriatric population in skilled nursing, assisted living, and sub-acute facilities.
Responsibilities:
- Conduct individual and group therapy sessions, assessing, diagnosing, and treating emotional and behavioral health conditions such as depression, anxiety, and cognitive decline.
- Develop and implement evidence-based treatment plans in collaboration with facility staff, families, and interdisciplinary teams.
- Maintain accurate clinical documentation using a user-friendly EHR system, monitor patient progress, and adjust treatment as needed.
- Work within regulatory standards and provide compassionate, patient-centered care in a supportive environment.
Qualifications & Skills:
- Active, unrestricted LMFT license in the applicable state.
- Effective communication skills, organizational ability, and proficiency with electronic health records.
- Ability to provide in-person services within a 45-minute radius, with physical mobility to move throughout facilities.
- Full vaccination and booster may be required in some settings.
