The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, and family counseling, as part of an integrated primary care team.
Conducts assessments, diagnoses, develops treatment and self-management plans, and makes referrals. Supports patients with behavioral health, crisis intervention, and health management strategies. Collaborates with medical providers and community partners to enhance patient care. Documents sessions and care plans in EHR, ensuring confidentiality and compliance. Participates in team meetings, training, and quality improvement initiatives. Works in person, with some travel, in a culturally competent manner, adhering to organizational policies. Requires a Masters level in social work or related field; preferred licensure and experience. Day shift preferred.
