Licensed Clinical Social Worker

Position Overview: A Licensed Clinical Social Worker (LCSW) is sought to join a behavioral health team, providing direct clinical services to adolescent and adult patients in primary care and school-based settings.


Key Responsibilities:

  • Conduct psycho-social assessments and develop personalized treatment plans.
  • Offer individual, family, and group therapy based on comprehensive evaluations.
  • Coordinate therapy sessions and deliver crisis counseling and intervention when needed.
  • Collaborate with interdisciplinary team members and support clinical goals related to mental health.
  • Perform screenings for depression, anxiety, substance use, and related issues.
  • Ensure accurate documentation in the Electronic Health Record, maintaining confidentiality.
  • Refer patients for psychiatric evaluation, hospitalization, or in-home services as appropriate.
  • Communicate effectively with care partners to optimize patient outcomes.

Qualifications & Skills:

  • Master’s degree in social work from an accredited program.
  • Valid LCSW license in the relevant state.
  • Minimum of five years of social work experience.
  • Bilingual in English and Spanish is preferred.
  • Strong organizational, communication, and clinical documentation skills.
  • Knowledge of child and adolescent development and high-risk psychosocial factors.
  • Ability to build trusting relationships with youth and families.
  • Proficiency with Electronic Health Records and Microsoft Office.
  • Effective active listening, interpersonal skills, and sound judgment.
  • Commitment to ethical standards and ongoing professional development.

Additional Requirements: Completion of Mandated Reporter Training, CPR, and Infection Control certifications. Ability to work effectively in a fast-paced, team-oriented environment.
