Position Overview: A Licensed Clinical Social Worker (LCSW) is sought to join a behavioral health team, providing direct clinical services to adolescent and adult patients in primary care and school-based settings.
Key Responsibilities:
- Conduct psycho-social assessments and develop personalized treatment plans.
- Offer individual, family, and group therapy based on comprehensive evaluations.
- Coordinate therapy sessions and deliver crisis counseling and intervention when needed.
- Collaborate with interdisciplinary team members and support clinical goals related to mental health.
- Perform screenings for depression, anxiety, substance use, and related issues.
- Ensure accurate documentation in the Electronic Health Record, maintaining confidentiality.
- Refer patients for psychiatric evaluation, hospitalization, or in-home services as appropriate.
- Communicate effectively with care partners to optimize patient outcomes.
Qualifications & Skills:
- Master’s degree in social work from an accredited program.
- Valid LCSW license in the relevant state.
- Minimum of five years of social work experience.
- Bilingual in English and Spanish is preferred.
- Strong organizational, communication, and clinical documentation skills.
- Knowledge of child and adolescent development and high-risk psychosocial factors.
- Ability to build trusting relationships with youth and families.
- Proficiency with Electronic Health Records and Microsoft Office.
- Effective active listening, interpersonal skills, and sound judgment.
- Commitment to ethical standards and ongoing professional development.
Additional Requirements: Completion of Mandated Reporter Training, CPR, and Infection Control certifications. Ability to work effectively in a fast-paced, team-oriented environment.
