We are seeking a Licensed Addiction Counselor (LAC) to join our Community Health Center, providing outpatient primary care and behavioral health services.
Summary: The LAC will integrate Substance Use Services into the primary care setting, assist in developing and implementing these services, and offer care coordination and support to patients.
Responsibilities include:
- Collaborating with medical providers to identify and refer behavioral health needs
- Conducting Substance Use Assessments
- Developing treatment plans and coordinating care
- Serving as a resource on Substance Use issues
- Providing care coordination and oversight for Medication Assisted Treatment (MAT) and mandated treatments
- Delivering individual, group, and family treatment based on evidence-based practices
- Maintaining accurate clinical documentation and reports
- Participating in meetings and training
Qualifications: Montana LAC license, at least 2 years of clinical assessment and treatment experience preferred, and ability to commute/relocate to Hamilton, MT.
