Responsibilities:
- Perform comprehensive assessments and manage symptoms and pain effectively.
- Administer medications and develop individualized care plans.
- Collaborate with physicians, social workers, and spiritual care providers to support patients and families.
- Educate patients and families about care processes and expectations.
- Ensure accurate documentation and compliance with hospice standards.
