Responsible for providing safe, appropriate pharmacy services for home infusion patients.
Key duties:
- Review medication orders and ensure accuracy and suitability for patient needs.
- Perform sterile compounding and safe dispensing of home infusion medications.
- Develop patient-specific care plans, including medication monitoring and counseling.
- Monitor medication therapy and communicate clinically relevant information to patients and healthcare professionals.
- Supervise pharmacy personnel and support safe, compliant pharmacy operations.
- Manage medication and supply inventory to support ongoing infusion needs.
- Ensure compliance with USP standards and applicable regulations.
Qualifications/skills: Strong clinical pharmacy judgment, expertise in sterile compounding, accurate order verification, medication monitoring and patient counseling skills, leadership in supervising pharmacy staff, inventory management, and thorough knowledge of USP and regulatory compliance.
