Role: In-Home Nurse Practitioner
Responsibilities:
Perform comprehensive health evaluations for adults and seniors in their homes.
Conduct detailed assessments, review medications, and manage chronic conditions.
Identify gaps in care and utilize mobile technology and data-driven tools.
Collaborate with interdisciplinary teams to ensure coordinated care.
Document findings in the electronic health record (EHR) system.
Support value-based care initiatives and follow-up care planning.
Qualifications & Skills:
Active Nurse Practitioner license.
Strong assessment and clinical management skills.
Experience with chronic disease management and medication review.
Proficiency with mobile technology and data documentation tools.
