Role: Nurse Practitioner providing in-home and community-based health assessments for diverse patient populations.
Key Responsibilities:
- Perform comprehensive physical exams and review patient medical histories
- Assess, diagnose, and manage chronic conditions
- Document clinical findings and care plans in EHR systems
- Coordinate with physicians, care managers, and health plan stakeholders
- Support efforts to close care gaps and improve care quality initiatives
- Contribute to innovative care models that improve patient outcomes
Qualifications & Skills:
- Active Nurse Practitioner licensure and clinical competence in primary/acute care assessment
- Strong clinical judgment for chronic disease management
- Accurate documentation and proficiency with EHR workflows
- Excellent communication and collaboration skills with multidisciplinary teams
- Ability to work effectively with diverse populations in community settings
