Role: Hospice – Registered Nurse Case Manager
Responsibilities: Assess patient needs, develop and implement personalized care plans, collaborate with the healthcare team, and communicate updates. Promote respectful, patient-centered care addressing emotional, spiritual, and cultural needs. Evaluate patient condition changes, update care plans accordingly, and ensure evidence-based practices for quality outcomes. Responsible for coordination and advocacy in a safety-sensitive role.
Qualifications & Skills: Nursing Diploma or Associate’s degree, RN licensure, valid driver’s license, less than 2 years of experience (preferred Bachelor’s degree and 2-5 years experience). Emphasis on cultural competence, critical thinking, and safety awareness.
