Role Overview:
This position involves providing specialized nursing care to patients in a hospice setting, focusing on pain and symptom management, and supporting their emotional, spiritual, and cultural needs. The nurse will assess patient preferences, develop and implement individualized care plans, and collaborate with an interprofessional team, patients, and families to ensure quality, evidence-based care. Responsibilities include evaluating patient conditions, updating care plans, and advocating for patient-centered continuity of care in a safety-sensitive environment.
Qualifications & Skills:
- Associate’s degree or higher in Nursing (RN license required)
- Minimum of 2 years of nursing experience; hospice or home health experience preferred
- Valid driver’s license
- Strong critical thinking, communication, and cultural competency skills
- Ability to work safely and maintain alertness at all times
Key Duties:
- Assess patient needs and barriers, including cultural and emotional factors
- Develop, document, and communicate personalized care plans
- Collaborate with the healthcare team to deliver compassionate care
