- Triages after-hours calls and determines appropriate next steps.
- Performs urgent visits to assess patient condition, symptoms, and needs.
- Administers medications and implements individualized care plans focused on comfort, dignity, and quality of life.
- Collaborates with an interdisciplinary team to coordinate safe, high-quality end-of-life care.
- Educates and supports patients and families, addressing questions and care needs.
- Documents clinical findings and interventions in the EMR.
- Coordinates with physicians to ensure timely, appropriate care and symptom management.
- Works independently in the field using strong clinical judgment and problem-solving.
Qualifications/Skills: Registered Nurse license, strong clinical judgment, excellent communication and interpersonal skills, ability to assess urgently, provide compassionate care, and effectively document and coordinate with the care team.
