Role: Hospice RN On Call
Responsibilities:
- Provide after-hours nursing care to patients in their homes
- Triages calls and makes urgent visits as needed
- Manage patient symptoms and support end-of-life care
- Collaborate with an interdisciplinary team to ensure personalized care
- Document care accurately and follow individualized care plans
