Skilled Nursing Home Health Role
Provide and coordinate professional skilled nursing care for home health patients.
Key Responsibilities
- Assess patient needs and develop, implement, and update individualized plans of care.
- Monitor patient response to treatments and adjust care as clinically indicated.
- Document visits and maintain accurate clinical records.
- Communicate with physicians and interdisciplinary care teams to support coordinated care.
- Assist with discharge planning and promote continuity of care.
Required Qualifications & Skills
- Licensed/registered nurse able to provide skilled nursing services.
- Strong clinical assessment, care planning, and patient monitoring skills.
- Excellent documentation and clinical record accuracy.
- Effective communication and collaboration with healthcare providers.
- Ability to support discharge planning and continuity of care.
