Role: Registered Nurse (RN) – Home Health
Responsibilities:
- Provide skilled nursing care in patients’ homes, including assessments, wound care, medication management, and patient education.
- Develop individualized care plans tailored to each patient’s needs.
- Coordinate with physicians and interdisciplinary teams to ensure comprehensive care.
- Accurately document all care activities in electronic health records.
- Ensure compliance with clinical standards and quality protocols.
- Support patient independence and promote positive health outcomes.
- Contribute to initiatives aimed at improving patient satisfaction and overall care quality.
Qualifications & Skills:
- Current valid RN license.
- Strong clinical assessment and wound care skills.
- Excellent communication and coordination abilities.
- Attention to detail in documentation and compliance.
- Ability to work independently in a home setting and as part of a team.
