Role: Registered Nurse for home health services supporting a pharmaceutical patient population.
Responsibilities include:
- Providing compassionate, evidence-based nursing care in patients’ homes
- Managing chronic diseases, administering medications, and supporting post-acute recovery
- Performing patient assessments and developing individualized care plans
- Educating families and caregivers on disease management and treatment
- Documenting patient outcomes and care activities
- Collaborating with a research-driven, multidisciplinary team to ensure safe, high-quality care
Qualifications & Skills:
- Current RN license and relevant clinical experience
- Strong assessment, communication, and educational skills
- Ability to work independently in a home environment
- Commitment to clinical excellence and patient safety
- Experience with chronic disease management and medication administration preferred
